Yorkshire Health Network is excited to announce that the ProActive Care Team (PACT) will be launching in March 2022.
The PACT is an exciting new local service set up to empower patients to live better for longer and increase access to healthcare for all in Harrogate and the surrounding towns and rural areas.
The PACT will be ‘ProActively’ contacting patients that the local GP practices have identified as potentially needing extra support to access health care. We will reach out to people who are unable to leave their own home due to a medical condition as our first priority group.
We have three experienced Occupational Therapists, supported by our care coordinators, who will be working closely with the team at the GP practices.
Occupational Therapists can help people of all ages overcome difficulties caused by ageing, illness or accident so that they can carry out everyday tasks or occupations. The Occupational Therapist will consider all of the person’s needs – physical, psychological, social and environmental. This support can make a real difference giving people greater independence, choice and control over their health and future.
The PACT could help somebody:
Make everyday tasks in their home easier so that they can remain independent
By giving hints, tips and tools to manage mild-moderate memory loss
Achieve their goals
Make their home safer for the future
Increase their self-confidence if they feel anxious or low in mood
Avoid isolation and loneliness
Find better ways to manage household chores and daily tasks
Take part in hobbies that they enjoy and rediscover old interests
Connect to groups in their area who could help them.
